- Register by entering your name, date of birth, and email address. The email address needs to be the same as on your insurance card or Medicare card. You will receive an activation code via email after submitting the application form.
- To finish setting up your account, please use this code to access the next page where you will enter additional information including phone number, medical provider’s name, and location.
- Once completed, you can start using your CVS Health MyChart Account! We offer many ways to help manage your healthcare and make getting the care you need easier. With CVS Health MyChart, you have all of your personal health information in one place so it’s always at your fingertips.

How to Log In to Your Account
Logging into your CVS Health MyChart Account is quick and easy. Here are some instructions for logging in:
- Visit the CVS Health website
- Click on MyChart
- Enter your username and password in the appropriate fields, then click Login. If you don’t know your account information, contact customer service by clicking Contact Us at the top of any page on our site.
- After logging in to your account, you can manage all aspects of your care, including viewing lab results or scheduling a refill or appointment with a provider.
- You can also add family members to your account so that they can view certain health information as well. In order to do this, follow these steps:
- Select Manage Family Members
- Search for the person’s name in the search box
- Select Add next to their name
- Now that they’re added, you’ll see them under ‘Managed Family Members’.
How to Recover Your Username or Password
If you know your username and password, log in to the CVS Health Mychart website. Click on Forgot Username or Password and enter the email address used to create your account. You will receive a temporary password by email. Enter this password on the login page and your username should appear. If you do not have access to the email account associated with your account, please contact customer service for help recovering your information at 1-855-812-5206.
If you are unable to log in using either of these methods, it is possible that someone else has accessed your account without permission or there may be an issue with our system.
How to Use the MyChart Mobile App
It’s easy to use the CVS Health MyChart Mobile App. The app lets you access your medical records, pharmacy information, and lab results, right from your phone. Here are some tips for getting started with the app.
-Visit the Apple App Store or Google Play Store on your phone and search for CVS Health.
-When you find the CVS Health Mobile App, click Install.
-When the installation is complete, open up the app and enter your email address and password to log in (if you already have a mychart account). If you do not have a MyChart account yet, click Create an Account to create one.
After clicking create an account, select the option I am currently active at this site and then choose Create New Account. You will need to provide a username, password, and email address. Then input your personal data such as name, date of birth, etc., after which you can select whether you want to share your information with doctors at other sites that participate in MyChart if desired.
